2015 Tax Credit Form

Use this form to support your student through a tax credit donation. Be sure to follow the instructions on the form. Checks must be payable to Gilbert Public Schools and the NAME on the check must match the NAME on the Form. Cash, Cashier's Check, or a Money Order are also acceptable forms of payment to turn in with the tax credit form.
